Application for Learner’s License

As per the Central Motor Vehicles Rules (CMVR), 1989, an application for a learner’s license must be made in Form 2 and should be accompanied by:

Once the learner’s license period is completed, an application for a permanent license must be made in Form 4, along with: 

  • Valid learner’s license
  • Medical certificate (if applicable)
  • Three recent passport-size photographs
  • Appropriate fee as per Rule 32
  • Proof of residence & age
  • Successful completion of a driving test

Renewal of Driving License

A driving license is valid for a specific period and must be renewed before expiry.

Application for Renewal

As per CMVR, 1989, an application for renewal must be submitted in Form 9 to the concerned RTO and should include:

  • Appropriate fee as per Rule 32
  • Three recent passport-size photographs (if renewal is done in Form 6)
  • Existing driving license
  • Medical certificate (if applicable)

Issue of Duplicate Driving License

A duplicate driving license is issued in case of loss, theft, or damage of the original license.

Application for Duplicate License

As per CMVR, 1989, the process includes :

  • Immediate intimation to the licensing authority regarding the loss or damage 
  • Submission of a written application for a duplicate license
  • Appropriate fee as per Rule 32
  • The duplicate license will be marked as “Duplicate”
  • If the original license is found later, it must be surrendered to the RTO

Note : Filing an FIR with the police may be required for lost licenses.


Change in Address on Driving License

If you have moved to a new residence, it is mandatory for you to update the address on your driving license.

Application for Address Change

As per CMVR, 1989, an application must be submitted in Form 33 to the concerned RTO and should include:

  • Application in Form 33
  • Original driving license
  • Proof of new address (such as Aadhaar card, utility bill, rent agreement, etc.)
  • Appropriate fee as per Rule 32
  • Three recent passport-size photographs
